Welcome to Affiance Partners! Now that you have received your Welcome Packet, we are sure that you will have some questions regarding the use of this program. Our goal is to assist you with our processes and make this transition as smooth as possible. Please take a few minutes to review the paperwork that was sent to you. In your Welcome Packet you should have received an identification card which identifies you as an Affiance Partners client. At your next physician/provider appointment, simply present this card to the billing department. They can contact our office at the telephone number listed on the card for any specific questions they may have. Please be sure to return the documents that have been included in your Welcome Packet. This will allow us to notify your providers of your MSA and the release of information will allow us to obtain necessary claims information for processing of your medical claims.
If your MSA included pharmacy coverage, it is noted on the front of your identification card. We have partnered with a large cost containment company to help insure that you are paying the loewst prices for your pharmaceutical and medical supplies. Your name and contact information will be forwarded to them and they will in turn, contact you to discuss your needs. In a few short days, you will receive an identification card in the mail that will provide access to over 59,000 pharmacies nationwide or, should you choose to participate in an even greater cost savings, utilize a mail order option.
If you have paid for medical expenses out of pocket, all that is required is for you to forward your receipts to our office for processing (please tape small receipts to a larger sheet of paper). Please be sure that the receipt includes the amount paid, the provider of the service and the date the payment was made. Unfortunately, we are unable to process bills that reflect "Payment on account". Payments must show an actual date of service and to whom payment was made. |
